Maternity Leave

Hello, I was filling out the application for Maternity leave and I have some questions:
- My Record of Employment was submitted as bi-weekly pay periods (27) , but they ask for the highest best weeks in the last 52 weeks so would I have to divide the bi-weekly amount$ by two to get a single week ? Is it really necessary to report all of this, or they can just calculate my best weeks based on the record of employment submitted?
- What percentage of taxes should they take off so I won’t have to own them anything for my income taxes? I know there has been many people complaining about having to pay money back because they didn’t got enough taxes off, but what % is enough so this won’t happen?
