birth certificate application

Okay, so my boyfriend and I are going on a trip in January, and he needs his passport. He went into the federal building yesterday to apply, and they said because his birth certificate is torn (still readable if you lay it flat, but not acceptable to them I guess) he needs to apply for a new one. She gave him a form to fill out and fax away, and for rush delivery (48 hours she said), it is $60. So that's fine, he comes home and fills it out, but I was wondering if we need to send the old one with the application, so I went online to see if I could find some information. Using a Google search, I came to an application form from Express Legal, which was not the same form he was given at the federal building, but found using the search words "Manitoba Vital Statistics", the title on his original form. This form looks legit, but it states the price as being $72.25, PLUS another $35 for rush delivery. It also said it would be 25-30 days for processing. We decided to call the toll-free number for information, but noticed that the numbers on each form were not the same. I then noticed that this new form I found was to be sent to Nelson, BC. Well, this can't be right, I thought. Why would we send an application for a Manitoba birth certificate (which is what this one seemed to be for) to BC? Needless to say, we stuck with the form he was handed in person.
So, could this be a scam of some sort? Not only to get your money, but to also get your personal information? Seems scary, as anyone else who is looking online for this form may not have another one to compare it to, and this one did seem very official, even stating all of the Manitoba security features on this new card (which by the way, this website said all new certificates will be a plastic card instead of paper, is this right?).
