Growing root vegetables, I need help

would some one Please tell me why we can't grow root vegetables in our garden. Anything above ground looks and produces just fine but anything below ground is really embarassing. Our carrots grew to a whopping 3 inches long and our potatoes produced about 2 per hill [and those were even kinda scabby]. What's wrong with our soil. The soil is a very sandy base but we have been adding leaves, lawn clippings and well rotted manure every year for 3 years now and using the rotor tiller spring and fall. WHAT am I doing wrong??
