Poor cat (bladder infection)

So my cat has a bladder infection, he has already been to the vet once and got pills and new food. The previous vet said that he didnt have stones, but a lot of crystals.
But now he's meowing a terrible sound and he's straining really bad. I don't think he can urinate at all...
I wonder if this is the type of thing the on-call vets are for (I know I would pay a whole bunch).
Does anyone know?
