I'm not sure you are correct when you say that you need to get to a doctors office to be refereed by an OT before they can'go through the motions' with SMD.
If you have the chair, then you have already been referred and found eligible for the chair. You don't need a doctor referral again.
Simply call SMD directly and let them know about the situation. They'll want to know about their equipment, and trust me people break this kind of equipment all the time, so they have a plan in place.
It does belong to SMD so they have a right to know that's it's broken and to follow their set terms of replacement.It would be best to call them first, then they will tell you how long of a delay to expect.
At that time, you would call either your physician or OT at the hospital and explain that your situation was desperate because of lost wages. They would begin to search for an available wheelchair and i know there are definitely chairs available for short term use.

I'm sure we can all understand why every person eligible for a free wheelchair can't be given 2 wheelchairs just in case something happens at that cost...someone is paying. Instead, they have a replacement system set up for when things go wrong.
