Full Time Term

Position Summary  

The Case Manager for the Sioux Valley Dakota Nation Jordan's Principle Program is responsible for delivering comprehensive, culturally grounded case management services to First Nations children and families. This role ensures timely, equitable access to health, education, social, mental health, respite, and specialized supports under Jordan's Principle.

The Case Manager serves as the central liaison between families, schools, healthcare providers, service agencies, Indigenous Services Canada, and Sioux Valley Dakota Nation departments. The position combines direct client support with program administration, budgeting oversight, reporting, staff coordination, community engagement, and advocacy.

The incumbent must uphold Dakota values, traditions, and community priorities while ensuring all services meet Jordan's Principle guidelines, funding requirements, and legislative standards.

Key Responsibilities 

1. Case Management & Client Support

  • Intake, assess, and process referrals for Jordan's Principle services.
  • Develop individualized service plans with families, Elders, schools, and providers.
  • Coordinate access to medical, mental health, educational, cultural, respite, and specialized supports.
  • Maintain ongoing communication, monitor progress, and respond to crises.
  • Advocate for equitable, timely services and assist families with documentation.
  • Ensure all supports are culturally safe, trauma-informed, and family-centered.

2. Program Administration

  • Manage funding requests and ensure compliance with ISC policies.
  • Maintain accurate records, case notes, and tracking systems.
  • Prepare requisitions, payment requests, and service agreements.
  • Support policy development, quality assurance, and operational improvements.

3. Budgeting & Financial Oversight

  • Monitor program budgets and expenditures.
  • Track allocations, prepare financial summaries, and support annual budget planning.
  • Identify funding gaps and recommend resource strategies.

4. Reporting & Documentation

  • Prepare monthly, quarterly, and annual reports.
  • Compile statistics, outcomes, and performance indicators.
  • Support audits, evaluations, and funding reviews.

5. Staff Coordination & Meeting Facilitation

  • Lead staff and case management meetings.
  • Assist with caseload distribution, workflow coordination, and scheduling.
  • Provide mentorship, support conflict resolution, and encourage professional development.
  • Participate in recruitment, onboarding, and training.

6. Community Engagement & Advocacy

  • Build relationships with families, schools, healthcare providers, ISC, Elders, and community organizations.
  • Represent the program at meetings and events.
  • Conduct outreach and promote culturally grounded programming.
  • Advocate for systemic improvements affecting First Nations children.

7. Policy Compliance & Confidentiality

  • Ensure adherence to Jordan's Principle directives, ISC requirements, privacy laws, and SVDN policies.
  • Maintain strict confidentiality and follow informed consent procedures.
  • Report child safety concerns according to legislation.

8. Knowledge & Skills

  • Strong understanding of Jordan's Principle and First Nations governance.
  • Skilled in case management, crisis intervention, advocacy, and communication.
  • Knowledge of trauma-informed and culturally safe practices.
  • Strong organizational, reporting, and documentation abilities.
  • Experience with budgeting and program administration.
  • Proficiency in Microsoft Office, databases, and electronic filing systems.

Working Conditions

  • Work occurs in office, community, school, home, and travel settings.
  • Some evening or weekend work required.
  • Exposure to sensitive and emotionally demanding situations.
  • Fast-paced environment with competing priorities.

Requirements

  • Diploma or Degree in Social Work, Human Services, Indigenous Studies, Nursing, Community Development, Public Administration, or related field (or equivalent experience).
  • 3?5 years experience in case management, Indigenous health/social services, Child & Family Services, or Jordan's Principle administration.
  • Valid Class 5 Driver's License and reliable vehicle.
  • Clear Criminal Record Check and Child Abuse Registry Check.
  • First Aid/CPR may be required.
  • Experience working with First Nations communities is required.
  • Budgeting, reporting, and supervisory experience are assets.
